If you are sick, have recently traveled, or been exposed to COVID directly, we ask that you stay home and notify us for rescheduling of your appointment.
If you have had your full vaccination, please bring a copy to your appointment and we will scan it into your medical record file. This will eliminate having to answer our COVID-19 Questionnaire at your visit.
Please allow 72 business hours for refills, call backs and responses to Prior Authorizations and Referrals as pharmacies, insurances and other companies, including the postal service are making changes to their hours and turnaround time to our responses, too.
